Client Care Coordinator

At Senior Helpers of Northeast Wisconsin, our mission is to provide comfort, dignity, and independence to the clients we serve. We currently have three openings for Client Care Coordinators in our Fond du Lac, Green Bay, and Appleton offices.
As a Client Care Coordinator, you will be the bridge between our clients, their families, caregivers, and healthcare professionals—ensuring that every individual receives the highest quality of care tailored to their unique needs. This role goes far beyond scheduling; it is about advocating for clients, nurturing relationships, and ensuring seamless communication across all parties involved in care.
This position is best suited for a compassionate, service-oriented professional with strong organizational skills and the ability to manage multiple priorities. Success in this role requires a balance of empathy, operational efficiency, problem-solving, and leadership.
Key Responsibilities
Client & Family Advocacy
- Serve as the primary point of contact for clients and families, ensuring their needs and concerns are heard, addressed, and resolved.
- Build strong, trusting relationships while providing guidance in navigating senior care.
- Conduct comprehensive Care Consultations with prospective clients to create customized care plans.
Care Coordination & Oversight
- Create, implement, and regularly update individualized care plans.
- Schedule and match caregivers thoughtfully, based on compatibility, skills, and client preferences.
- Facilitate caregiver introductions to ensure smooth transitions.
Collaboration & Communication
- Serve as a liaison between clients, caregivers, healthcare providers, and families.
- Support and guide caregivers to resolve challenges and ensure adherence to care plans.
- Build and maintain strong relationships with referral sources, healthcare professionals, and community partners to strengthen Senior Helpers’ presence.
Qualifications & Preferred Background
- Bachelor’s degree or training in Healthcare Administration, Social Work, Gerontology, or related fields (preferred).
- Experience in client services, healthcare coordination, senior care, or case management.
- Knowledge of sales, community engagement, or referral development is a plus.
- Proficiency with Microsoft Office; prior experience with Wellsky or similar scheduling/documentation systems preferred.
