About Us:
Homewatch CareGivers of Yorba Linda is preparing to launch a licensed home health division to complement our established non-medical home care services. Our mission is to deliver expert, compassionate, and person-centered care that helps clients maintain their independence and dignity at home. We are seeking an experienced and credentialed RN to serve as our Director of Patient Care Services (DPCS) during this exciting new phase of growth.

Position Overview:
The DPCS will be responsible for overseeing all clinical and patient care services in compliance with the State of California regulations for home health agencies. This RN leader will help establish and maintain the clinical foundation for the agency, provide direct oversight of skilled and non-medical services, and assist in training, evaluations, client care, and supervising clinical and non-clinical staff.

Key Responsibilities:

  • Direct and coordinate patient care activities for the home health and non-medical home care programs.
  • Conduct initial and ongoing patient assessments and create individualized care plans.
  • Along with our staff, you will oversee and participate in the training, orientation, and evaluation of caregiving staff.
  • Ensure clinical documentation and client records are audit-ready and in full regulatory compliance.
  • Participate or oversee the quality assurance and performance improvement (QAPI) activities.
  • Be available on-site or by telecommunications during hours in which patients are receiving services.
  • Support the development of a hands-on caregiver training lab to enhance in-house education.
Required Qualifications (Per Title 22):
The candidate for this position must meet one of the following requirements:

  1. RN with a bachelor’s (or higher) degree in nursing or a health-related field, and three years of experience within the last five years in a home health agency, primary care clinic, or health facility, with at least one year in a supervisory or administrative role;
    OR
  2. RN with four years of experience within the last five years in a home health agency, primary care clinic, or health facility, with at least one year in a supervisory or administrative role.
Additional Requirements:

  • Active California RN license in good standing
  • CPR/First Aid certified
  • Valid driver’s license and reliable transportation
  • Strong clinical assessment, documentation, and teaching skills
  • Passion for elevating care and mentoring care teams
Schedule & Pay:

  • PRN/part-time to start 
  • Flexible hours; field-based and remote documentation
  • Pay rate dependent on experience; to be discussed during the interview process
Preferred Candidates May Include:

  • Retired or semi-retired RNs seeking to work flexible hours
  • RNs that are currently working that are seeking flexible supplemental hours
  • RNs with home health leadership experience looking to help build and shape a growing agency

Compensation: $60.00 - $80.00 per hour
