About the Role
ComForCare Home Care - Yorba Linda is looking for a compassionate, organized Registered Nurse to lead client assessments, care planning and caregiver oversight for our in-home care clients in Anaheim Hills, Yorba Linda, Anaheim and nearby communities. You’ll be the clinical eye behind our non-medical care, making sure every client gets the right care plan and the right caregiver so they can live safely and independently at home. If you’re an RN who wants meaningful one-on-one time with clients and families, without the pressure of a high-volume clinical caseload, we want to hear from you!
Responsibilities:
- Conduct in-home assessments of new clients, including health history, functional abilities, cognition, fall risk and home safety
- Develop personalized, non-medical care plans and update them as client needs change
- Help match clients with caregivers whose skills and experience fit their needs, including dementia, mobility and end-of-life support
- Make regular supervisory visits to observe caregivers, check on client well-being and confirm care plans are being followed
- Train and coach caregivers on topics such as safe transfers, dementia care, infection control and recognizing changes in condition
- Educate clients and families on safety, fall prevention and what to expect as needs progress
- Communicate with clients’ physicians, hospice teams, home health agencies and other providers as needed to coordinate support
- Follow up on reported changes in a client’s condition and recommend next steps or referrals
- Document assessments, care plans and visits accurately in our scheduling and care management system (SwyftOps)
- Support compliance with California Home Care Services Bureau requirements and ComForCare standards
Requirements:
- Active, unrestricted California Registered Nurse (RN) license
- At least 1–2 years of nursing experience; experience in geriatrics, home care, home health, hospice, assisted living or care management is preferred
- Strong assessment skills and a practical understanding of aging, chronic conditions and dementia
- Warm, clear communication with clients, families and caregivers
- Organized and self-directed; comfortable working independently in the field
- CPR certification required; OASIS experience a plus
- TB clearance
- Valid driver’s license, proof of auto insurance and reliable transportation for home visits
- Comfortable using a smartphone or tablet for scheduling and documentation
