- Conduct client assessments and help develop, implement, and update individualized care plans.
- Maintain regular communication with clients and families to ensure services continue to meet their needs and expectations.
- Monitor the quality and appropriateness of care being provided and address concerns when they arise.
- Identify changes in a client’s condition, needs, or circumstances and communicate those changes to the appropriate individuals.
- Help ensure caregivers are appropriately matched and assigned based on each client’s individual needs.
- Support and mentor caregivers in delivering safe, compassionate, high-quality care.
- Investigate client concerns, service issues, incidents, policy violations, and other reportable events, and assist with appropriate follow-up and resolution.
- Maintain accurate and timely client records and documentation.
- Help clients and families identify community resources that may support continuity of care.
- Build positive relationships with referral partners and other professionals throughout the community.
- Represent Right at Home professionally at community events, meetings, and programs as needed.
- Work collaboratively with our office team to solve problems and create the best possible experience for our clients and caregivers.
- Is compassionate, personable, and comfortable working directly with seniors, adults with care needs, and their families.
- Has strong communication and relationship-building skills.
- Is highly organized and able to manage multiple clients, priorities, and deadlines.
- Uses sound judgment and is comfortable making decisions and solving problems.
- Can handle difficult conversations professionally and respectfully.
- Remains calm and positive in a fast-paced environment.
- Is comfortable working both in the office and out in the community.
- Has basic computer and office skills and is comfortable learning new systems.
- Has previous experience in home care, healthcare, senior care, case management, or a medical office environment; experience in these areas is strongly preferred.
- High school diploma or equivalent required.
- Minimum of two years of professional work experience.
- Valid California driver’s license.
- Reliable, insured personal vehicle and ability to travel throughout San Luis Obispo County for client visits and other business needs.
- Ability to meet applicable health requirements for employees who have direct contact with clients.
- Ability to read, write, speak, and understand English as required to perform the responsibilities of the position.
- Availability for occasional on-call responsibilities outside of normal office hours.
