About the Role:
ComForCare Home Health Care - West Bloomfield is looking for a compassionate and skilled LPN Nurse to join our dedicated team in West Bloomfield Township, MI. Responsibilities:
- Provide direct patient care in home health settings under the supervision of an RN or physician
- Administer medications, treatments, and wound care as prescribed
- Monitor and document patient health status, vital signs, and progress
- Develop and follow individualized patient care plans
- Communicate effectively with patients, families, and the broader care team
- Educate patients and caregivers on health management and best practices
- Ensure compliance with state regulations and company policies in home health care
Requirements:
- Active LPN license in the state of Michigan
- Previous home health, long-term care, or clinical nursing experience preferred
- Strong knowledge of medication administration and wound care protocols
- Excellent communication, documentation, and organizational skills
- Reliable transportation and valid driver's license
- Compassionate, patient-focused demeanor with a commitment to quality care
- CPR/BLS certification required or ability to obtain upon hire
