Role Overview:

Community Care Liaison is a key position within our home care agency. Primarily work in the field assessing clients, mentoring caregivers and building referral relationships. This role will educate healthcare providers, community organizations, and families about care services, ensuring a smooth transition for seniors and individuals with disabilities who need daily living assistance. This Community Care Liaison will primarily be covering Maricopa County. Office is based in Chandler, AZ.

The Community Care Liaison is the external face of the agency, responsible for outside sales, relationship building, and community outreach. The primary objective is to cultivate a robust referral pipeline while actively increasing market awareness. Role works in a team environment and reports directly to the owner.

Responsibilities

  • Represent Company: Face of the company in interactions with our care team members (caregivers), potential clients, current clients, client’s family members and referral sources
  • Referral Development Proactively identify, qualify, and develop long-term partnerships with referral sources, including hospital discharge planners, social workers, case managers, hospice, and skilled home health.
  • Territory Strategy: Develop and execute a localized sales plan designed to meet or exceed monthly and quarterly census goals.
  • Pipeline Management: Track outreach efforts, in-person visits, and follow-ups using agency software (WellSky)
  • Partner Education: Educate healthcare professionals, patients, and families on the scope of non-medical home care, caregiver matching, and respite programs.
  • Conduct In-Home Assessments: Build care plan based on assessment and work with Scheduler on matching a caregiver to the client
  • Document all assessments, follow ups and referral building activities in our operations system
  • Monitor Quality: Ensure quality of care via follow up calls with clients, in-home supervisory visits, engaging with caregiver(s) assigned to the case and referrals to wrap around skilled health services for best outcomes
  • Care Navigation: Know all local resources and how to connect our clients to services and resources that are out of our scope of practice
  • Main Support System: For clients, caregivers and referral sources with direct communication with owner
  • Work in a Team Environment: With Owner, Recruitment and Retention Director, Staffing Specialists and Caregivers
  • On-Call Responsibilities: Minimum of 1 weekend per month and occasional week night as needed
Qualifications

  • Must have 3 or more years of proven experience in social work, case management, home health, hospice AND/OR home care 
  • Compassionate, kind, caring and be a great listener
  • Outstanding problem-solving skills and ability to work in a fast-paced business
  • Understanding of working in a continuum of care industry
  • Excellent follow through skills and self-starter
  • Able to pass series of background checks, Level One Fingerprint Clearance Card, Proof of Negative TB, Current First Aid and CPR Card
  • Experience in the hospice B2B referral building field is a plus!

Pay: $55,000-$60,000 yearly
Compensation: $55,000.00 - $60,000.00 per year
