About the Role:
ComForCare Home Health Care (North San Diego) is looking for a compassionate and skilled LVN to join our dedicated home health team in San Diego, CA. This is a rewarding opportunity to make a real difference in patients' lives by delivering personalized, one-on-one care right in the comfort of their homes. Responsibilities:
- Provide skilled nursing care to home health patients in accordance with physician-approved care plans
- Perform wound care, medication administration, injections, and other clinical procedures
- Monitor and document patient health status, progress, and changes in condition
- Educate patients and family members on disease management, medications, and home care techniques
- Coordinate with RNs, physicians, and interdisciplinary care teams to ensure continuity of care
- Maintain accurate and timely clinical documentation in compliance with state and federal regulations
- Conduct home visits across the North San Diego service area on a scheduled basis
Requirements:
- Valid California LVN license in good standing
- Current CPR/BLS certification
- Prior home health, skilled nursing, or clinical experience preferred
- Strong knowledge of wound care, medication management, and patient assessment
- Excellent communication, documentation, and organizational skills
- Reliable transportation, valid driver's license, and auto insurance
- Compassionate, self-motivated, and able to work independently in a field-based setting
