About the Role:
ComForCare Home Health Care (North San Diego) is looking for a compassionate and organized Client Care Coordinator to join our growing team in San Diego, CA. In this role, you'll be the vital link between our clients, their families, and our caregiving staff — ensuring every person we serve receives the highest quality of care. Responsibilities:
- Coordinate and manage home care services for clients, ensuring seamless care delivery
- Serve as the primary point of contact for clients, families, and caregivers
- Conduct client intake assessments and develop personalized care plans
- Schedule and match caregivers to clients based on needs, availability, and compatibility
- Monitor client satisfaction and proactively address concerns or changes in care needs
- Maintain accurate client and caregiver records in compliance with home health care regulations
- Collaborate with healthcare professionals and referral sources to support continuity of care
Requirements:
- Prior experience in home health care, case management, or a related healthcare coordination role
- Strong interpersonal and communication skills with a client-first mindset
- Excellent organizational and multitasking abilities in a fast-paced environment
- Proficiency with scheduling software and basic computer applications
- Knowledge of home care regulations and compliance standards preferred
- Empathetic, dependable, and solutions-oriented team player
- High school diploma required; Associate's or Bachelor's degree in healthcare or related field a plus
