About the Role:
ComForCare Home Health Care (North San Diego) is looking for a compassionate and skilled Private Duty Nurse (PDN) to join our dedicated team in San Diego, CA. This is a rewarding opportunity to make a meaningful difference in the lives of patients by delivering high-quality, one-on-one nursing care in the comfort of their own homes.
Responsibilities:
- Provide direct private duty nursing care to patients in a home health setting
- Assess, monitor, and document patient health status and progress
- Administer medications, treatments, and medical procedures as prescribed
- Develop and implement individualized patient care plans
- Communicate effectively with patients, families, and the care team regarding patient needs and updates
- Educate patients and caregivers on health management, medications, and safety
- Maintain accurate and timely clinical documentation in compliance with regulations
Requirements:
- Active LVN license in the state of California
- Prior experience in private duty nursing, home health, or pediatric care preferred
- Current CPR/BLS certification required
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively in a home setting
- Reliable transportation and valid California driver's license
