We are looking for a Client Care Liaison to join our team!

Key Responsibilities:
- Develop, implement, and manage individualized care plans that ensure the highest quality of in-home care
- Build and maintain strong relationships with clients, families, caregivers, and referral partners
- Serve as the primary point of contact to ensure care is safe, appropriate, and aligned with client needs
- Support and mentor caregivers to promote excellence in care delivery
- Act as a liaison within the local community to strengthen partnerships and expand awareness
- Maintain clear, consistent communication with all stakeholders
- Balance multiple priorities while maintaining a calm, positive, and solution-oriented approach
Qualifications:
- High school diploma or equivalent required; additional education or training is a plus
- Minimum of 2 years of professional experience in a related field
- Valid California driver’s license
- Reliable personal vehicle for in-home visits throughout San Luis Obispo County (mileage reimbursed)
- Availability for occasional on-call responsibilities outside normal business hours
