Responsibilities:
- Evaluate patients’ functional abilities and develop individualized care plans
- Provide therapeutic interventions to improve daily living skills
- Educate patients and caregivers on adaptive techniques and equipment
- Recommend home modifications to enhance safety and independence
- Document patient progress and adjust treatment plans as needed
Qualifications:
- Licensed Occupational Therapist (OT) in the state
- Home health experience preferred
- Strong clinical assessment and documentation skills
- Ability to work independently
- Reliable transportation
