Right at Home is a growing, locally owned home health company based in Cedar Rapids. We currently provide skilled nursing, companion and personal cares for a wide variety of clients in Eastern Iowa. We proudly provide these services to veterans, and we are in need of a part-time physical therapist to expand the level of skilled care available to them.
Responsibilities of a Physical Therapist include:
· Provide high quality evaluation and treatment of neuromusculoskeletal disorders
· Develop personalized care plans to improve mobility, strength, and overall function of patients in the home setting, involving patient in setting goals
· Communicate and collaborate with associated medical professionals to achieve patient goals
· Clear, concise documentation in our Clear Care software system
· Regular communication with physicians, nurses, case managers within Veterans Affairs as needed
· Ability to work independently and autonomously, managing one’s own schedule
· Supervise home health aides involved in patient care

Compensation: $65.00 - $67.00 per hour
