Position Summary:

At ComForCare Home Care Calgary, we support older adults and individuals with disabilities to live with dignity, independence, and comfort wherever they call home.
We are seeking a dedicated, compassionate and detail-oriented Licensed Practical Nurse (LPN) to join us part-time to conduct client health assessments, care, home safety checks, and reassessments.
Key Responsibilities:
- Conduct comprehensive health assessments for new clients
- Perform reassessments for existing clients as required
- Complete safety evaluations of client home environments
- Identify potential health risks and develop mitigation strategies
- Collaborate with caregivers, clients, and families to support safe care delivery
- Provide documentation, recommendations, and support for care plans
- Maintain accurate client health records in compliance with standards
- Support in developing personalized care recommendations
- Communicate findings clearly with the ComForCare team and stakeholders
Qualifications:
- Active Licensed Practical Nurse (LPN) license in good standing with CLPNA
- Minimum 2 years of nursing experience in home care, long-term care, or hospital settings
- Strong assessment and critical thinking skills
- Ability to work independently and communicate professionally
- Valid driver’s license and access to reliable transportation preferred
- Passion for senior care and person-centered service
- Excellent communication skills and attention to detail
