Description: Female client is 97 y/o, continues with poor vision and is legally blind due to Glaucoma, and continues with impaired hearing also. Client states she has no difficulty with soft foods and regular liquids, but states when she eats meats and coarse foods she must chew the foods thoroughly before swallowing to prevent any problems. Client denies any Falls in the past 6 months. Client now could benefit from Partial Assist with Showering and Nail and foot care, Applying Lotion, and Partial Assist with Dressing. Client continues to require services for assistance with ADL’s/IADL’s. Client requires Total assist with Household Management and with Meal preparation, and SBA with bathing, dressing, safety with Ambulation with walker, shopping and Transportation. **Caregiver must speak clear english, drive and cook.
Schedule: 10a-6p Monday-Friday
Payment: $16.00 an hour

Job Requirements:
Current HHA Certificate or CNA License
Valid Driver's License and current Auto Insurance
Level 2 Background Check
CPR Certified
Recent PPD or Chest X-Ray indicating negative for TB
Certificates in: Domestic Violence; Alzheimer’s; HIPAA; OSHA, Self-Administration of Medication
