HIRING IMMEDIATELYF for RN to join our management team as our Home Care Coordinator.

The Home Care Coordinator (HCC) will be an integral part of our management team. This high-profile position requires a dynamic professional with a commitment to excellence, a positive attitude and a true passion for helping others. 

The Home Care Coordinator will provide oversight of care plans, support services, and coordinate with other healthcare providers to ensure that clients receive the best possible care. You will also be responsible for community outreach and generating referrals from other healthcare professionals.

Responsibilities:

  • Initial and Supervisory Assessments of clients in their home
  • Develop and implement home care plans for clients
  • Monitor client progress and make necessary adjustments to care plans
  • Coordinate with other healthcare professionals, such as doctors, nurses, physical therapists, and social workers, to ensure clients receive the best possible care
  • Community outreach and education to develop home care referrals 
  • Maintain accurate and up-to-date records of client care plans and progress
  • Provide support and guidance to clients, family members and caregivers
  • Ensure compliance with all applicable regulations and laws
  • Caregiver training as needed

Qualifications:

  • Must have Active RN license in Georgia
  • Minimum of 3 years of home care experience, with at least 1 year in a supervisory role
  • Strong clinical skills and knowledge of home care practices
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Compassionate and empathetic attitude toward clients and their families.
  • Reliable transportation and valid driver's license.
  • Clear background check and drug screening.

Compensation: $72,000.00 - $90,000.00 per year
