Here at ComForCare Home Care, you will be working one-to-one with your patients, allowing you to take your time with each assessment and provide exceptional care you can be proud of.

Home care experience is not a requirement, but you must come with a great attitude and willingness to learn!

JOB DUTIES
  • Perform comprehensive in-home assessments. This will include a physical and psychosocial assessment, the compilation of a medication profile and client resources, and evaluation of the living environment. You will be instrumental in implementing fall precautions, educating aides/clients on the proper use of necessary supplies/equipment, and any other relevant safety measures.
  • Develop a fully individualized plan of care based on your assessment.
  • Communicate with caregivers and the clients/families of the clients to stay up to date on any changes in their condition.
  • Update the Plan of Care accordingly to reflect/ensure progress towards goals and outcomes. 
  • Provide ongoing education and in-field support to HHAs and PCAs.
REQUIREMENTS:
  • Current License as a Registered Nurse (RN) in New York State
  • CPR Certification
  • Knowledge of safety and infection control practices
  • HIPAA confidentiality 
WORK LOCATION:
  • Primarily in the field: Brooklyn and Manhattan, occasionally the Bronx and Queens 
SCHEDULE:
  • Flexible schedule 
SALARY: 
  • $50.00-$65.00 hourly

Compensation: $50.00 - $65.00 per hour
