Job Description: Provide RN oversight and guidance for Medicaid, Veteran and Private Pay programs.

Performs RN duties in accordance with our policy and procedures for all programs that are assigned.
Completes and participates in the assessment of patient needs and implementation of patient care.
Participates in patient care and planning meetings.
Notifies and documents change in condition of client(s).
Makes periodic rounds and/or calls to each assigned patient.
Visits client's homes to assess the needs for new patients.
Develops relationships with referral sources to assist in getting patients open for services.
Do supervisory visits to client's homes as required.

Qualifications:
Registered Nurse license in the state of WV
Associate's Degree
Excellent customer service skills
Ability to work independently
Must be able to develop relationships with case managers and social workers at the hospitals. Travel required in Kanawha and Putnam counties.
