Provide one on one assistance with the activities of daily living: Personal care, showering, grooming, dressing, oral hygiene, continence care, exercising, transferring, turning/re positioning, safety supervision, feeding, medication reminders, transportation and companionship. Other duties as assigned.

Requirements:

AZ DRIVERS LICENSE
FAMILY ATMOSPHERE
PROOF OF CAR INSURANCE (if wanting to run errands/doctor appts)
FINGERPRINT CLEARANCE CARD
CURRENT TB TEST RESULTS
CPR CERTIFICATION (will accept on-line training)
1ST AID CERTIFICATION (will accept on-line training)
6 MONTHS EXPERIENCE PREFERRED
3 PROFESSIONAL REFERENCES
